I've just finished reading an old book on early italian cinema. It seems that Eleonora Duse, while shooting a movie (Cenere - 1916. It can be found on youtube) asked a cello player to play while she shot her scenes.

Another one who wanted music on the set both in the silent and talkie era was John Ford. Actually he wanted Frank Borzage's brother to play tunes on the accordion not only while shooting some scenes but even when he was making his morning entrance on the set. (Source: Bogdanovich)
