Yeah, I think he's smiling because of his drug induced state but in all actuality he isn't happy at all and that is the only way he can escape, thus making everything seem very bittersweet.

He smiles because he knows he's not going back to jail because Max sapped him.

But raping her must've created barriers too...

I never thought he managed to rape her . I always thought the driver stopped him in time to do it.

No, Dlanor, Noodles gave Deb 300 million of his little friends, pretty clear.  Driver (Milchan) is thoroughly disgusted, does a good job refusing the dollars.

yeah... the idea that he thinks he can get with deborah after he raped her and she left town is absolutely ridiculous... gang or no gang... and i'm sure that the fact that he has a number of men trying to kill him now is a problem in the same way that running with his gang was before his gang died off... so that theory makes no sense to me... not even a little... to me there are only two reasonable theories... either he is escaping the horrors of his life with the opium and he can finally be in a happy place for a little while... or he simply is high on opium and smiling for no reason... or both

His smile has one reason: IRONY.

There are actually two questions here: why Noodles smiles at the end (as a function of the plot) and why Leone shows us Noodles smiling as the last image of the film. The first is easily answered and has already been done so on this thread (the man is buzzed!); the second, and more interesting question, could inspire the writing of any number of film studies dissertations.

I *would* like to observe, however, that that final image reminds me of the shot of Jill on her bed at Sweetwater photographed through the canopy........
