Il Monco actually offers to Baby a choice between being caught dead or alive, before being interrupted by the three Baby's accomplices.

For Smoker:

It is not the sheriff having a shave in the barbershop, but Baby's accomplice.

About Iceman hypothesis: Il Monco's answer it fits, Baby's less:  he is vaguely afraid of the stranger, otherwise he wouldn't have accepted his bold intrusion in the game.

 hola amigos. the other day I was watching the 'special edition' dvd of FOR A FEW DOLLARS MORE without the Frayling commentary and I noticed something - the scene when Manco shoots Cavanagh in the Saloon - for some reason, Morricone's 'Flute Sting' plays twice!  originally it was heard after he kills Cavanagh but here you first here it after he shoots 'Half-Shaved Face' & Co. and then after Cavanagh gets it. Why?! I think it ruins the scene... don't ask me why, it just does.
