There is one thing that really puzzles me with this movie: What is the meaning of Joe Pesci's second appearance (in the hospital)?

Joe Pesci is the symbol of Italian-American  mafia boss. He came at the hospital at the same moment when Democrate Party leader, wounded Syndicate leader and Jewish mob drunk champagne together in the bedroom.

I think Sergio showed him entering the hospital perhaps to prepare the plot  or  to confirmate Mr Berkovicz as a great hope  for mafia, future of political influence and business.

Its a good scene because Noodles cross with Pesci  without seeing him.

EXACTLY!We are given the possibility of the Mafia being involved in Max's faking of his death and later 're-birth' as Senator Bailey.Maybe Max told Noodles about his plan to rob the Federal Reserve Bank to get Noodles to think about turning Max in,thus setting up his faked death,etc.......

you lot may need to watch that scene again. The person Max and Noodles pass in the hospital isnt joe pesci, it just looks like him.
